HEALTH REQUIREMENTS AND FORMS

HEALTH REQUIREMENTS FOR GRADES 6

Illinois state law requires that all students have a complete physical examination performed by a licensed health care provider (MD, DO, APN or PA) and show evidence of the immunizations outlined below before entering 6th grade. District 29 has a policy of exclusion after October 15 of the current school year, per State Guidelines. Students who do not have a current physical exam and updated immunization records on file will be excluded from school until such requirement is met.



ALL 6TH GRADE STUDENTS MUST MEET THE FOLLOWING STATE HEALTH REQUIREMENTS IN ORDER TO ATTEND D29:

  1. PHYSICAL EXAMINATION: Students entering grade 6 must submit a physical examination dated within 12 months of the first day of school. 

 

  1. IMMUNIZATIONSMinimum Immunization Requirements. The following immunizations must be verified by the student's healthcare provider and turned in to the school nurse:

 

VACCINE

 

DOSE REQUIREMENT

Tetanus, diphtheria, pertussis  at least 4 doses of DTP/DTaP or Td; with the last dose on or after the 4th birthday
Tdap  1 dose of Tdap given on or after 11 years of age
Polio  at least 4 doses of polio vaccine with the last dose on or after the 4th birthday
MMR   2 doses of MMR vaccine. The first dose must have been received on or after the 1st birthday and the second dose no less than 28 days later
Varicella   2 doses of varicella. The first dose must have been received on or after the 1st birthday and the second dose no less than 28 days later
Hepatitis B  3 doses of hepatitis B vaccine administered at appropriate intervals
Meningococcal Disease  1 dose of meningococcal conjugate vaccine on or after 11 years of age

 

 

 

 

  1. DENTAL EXAM. In compliance with Illinois Public Act 093-0946, students entering grade 6 are required to have a dental exam on file. School dental examinations must have been completed within 18 months prior to the May 15 deadline. 

EXEMPTIONS

The State of Illinois allows for two kinds of exemptions. See below for information regarding each kind of exemption:

1) MEDICAL EXEMPTION: If your student has a medical contraindication for any of the required Illinois State Vaccines, please have your healthcare provider write a letter stating the following:

  • a list of vaccines that are medically contraindicated
  • the reason why they are medically contraindicated

Should the medical condition of the child later indicate that immunization is no longer contraindicated to the health of the child, the immunization requirements will then have to be met. Please submit this letter along with the physical before October 15 to avoid school exclusion. 


2) RELIGIOUS EXEMPTION: If you are requesting a religious exemption from any of the Illinois State required vaccinations the Illinois Certificate of Religious Exemption Form must be completed and signed by a physician. Please submit this form along with the Illinois Child Health Examination Form before October 15 to avoid school exclusion.

OTHER GRADE-LEVEL HEALTH REQUIREMENTS

  1. Students that are new to an Illinois School (from out of state or previously homeschooled), must also provide a current PHYSICAL EXAM dated within 12 months of the first day of school.
  2. Students that are new to an Illinois School (from out of state or previously homeschooled), must also provide a current VISION EXAM dated within 12 months of the first day of school. 
  3. All grade levels must meet the minimum immunization requirements per the Illinois Department of Public Health. Health files will be audited yearly to ensure compliance.

SPORTS REQUIREMENTS FOR STUDENT ATHLETES

  1. Students athletes must have a current school physical on file to play sports. A sports physical is accetable as well and is valid for 395 days. Students who do not have a current school physical will not be allowed to practice or play sports.   2. Concussion information is reviewed every year at registration. A concussion is a brain injury that affects how your brain works. It can happen when your brain gets bounced around in your skull after a fall or hit to the head. Please review the Fact Sheet for Athletes
    • If your child is diagnosed with a concussion, please let the school nurse know. Your doctor will provide a doctor’s note describing certain restrictions at school.
